FVBA LIABILITY WAIVER​

I/We the Parents/Guardians of the above named player desire to have him/her participate in the Clinic & Program activities, do hereby for myself and do hereby for my self and my heirs, executors and administrators agree to forever release, discharge, fully indemnify and save harmless Fraser Valley Baseball Academy Ltd. and their respective employees from any claim, demands, costs, liabilities, actions, losses of physical or mental injury or death at any time and in any manner arising in whole or in part directly or indirectly from participating in any of Fraser Valley Baseball Academy Ltd.’s clinics, and whether or not such damage or loss, physical  or mental injury or death is due to the negligence of any person whomsoever, including without limitation any member of Fraser Valley Baseball Academy Ltd and their employees.
